Friday Health Plans of Colorado’s Medicare Cost Plans are offered in 16 of the most rural and under-served counties in Colorado including Alamosa, Bent, Chaffee, Conejos, Costilla, Crowley, Custer, Fremont, Huerfano, Las Animas, Mineral, Otero, Prowers, Rio Grande, Saguache, and Yuma Counties.

Friday Health Plans of Colorado provides all Medicare-Covered Benefits. To qualify, you must reside in one of the above counties, be eligible for Part A, be enrolled in Part B and not have End Stage Renal Disease (ESRD). If you do have ESRD, you can still join our plan in certain situations. If you have had a successful kidney transplant, you can join a plan. For additional ESRD circumstances please call Customer Service at 719-589-3696 if calling from within the San Luis Valley or at 1-800-475-8466 if calling from outside the San Luis Valley. (TTY users should call 1-800-659-2656). You will still need to continue payment of your Part B premium. Our plans do NOT cover Part D or drug benefits.

You may enroll in our plan:

  • When you first become eligible for Medicare (three months prior to the month you turn 65 to three months after the month you turn 65)
  • If you become eligible for Medicare due to a disability (three months before and after you receive 24 months of cash disability benefits)
  • During annual Medicare open enrollment periods
  • At other times, due to special circumstances
  • And individuals enrolled in Original Medicare may chose to enroll in one of Friday’s Medicare Plans any time throughout the year

Friday Health Plans of Colorado is a health plan with a Medicare Contract. Enrollment in Friday Health’s Medicare cost plan depends on contract renewal.

You are eligible to enroll if you are entitled to Medicare Part A and/or enrolled in Medicare Part B and you live in the Friday Health Plans of Colorado’s service area. You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party. Eligible beneficiaries can enroll in Friday Health Plans of Colorado at any time.

You can use any doctor who is part of Friday’s network. You may also go to doctors outside of our network. We may not pay for services you receive outside of our network, but Medicare will pay for its share of charges it approves. You will be responsible for paying the Medicare deductible and coinsurance for those services, unless they were authorized in advance by Friday Health Plans of Colorado.
